ACTUAL CASH VALUE of what is selected below based on a minimum declared value of $2.50 per pound. 1) Carrier will pay to repair the item in order to restore it to the same condition it was in when it was received by Omega Moving & Storage, Inc or 2) pay the actual cash value of the property when it was received by Omega Moving & Storage, Inc. Carrier will apply depreciation to any claim made under this option. The charge of this option will be $7.50 per $1,000.00 of valuation, which requires ZERO DEDUCTIBLE.

REQUIREMENTS:

  • Inventory Sheet with value of all items moving submitted no less than 72 hrs prior to your moving date
  • Movers must verify all appliances or electronics to be covered are in working condition
  • An Omega Mover or your Moving Consultant may recommend items to be crated by wood for the proper protection while in transit; for example, high end TVs, all artwork, small motorized vehicles
  • Photos and Videos will be taken on the day of the move to provide confirmation of the condition of the items covered under the FVP plan

**Important Notice: Shipments are subject to a minimum policy of a declared value of $20,000.  Additionally, shipments valued over $100,000 will need to purchase an additional policy.

FULL REPLACEMENT VALUE of what is selected below based on a minimum declared value of $5.00 per pound. 1) Carrier will pay to repair the item in order to restore it to the same condition it was in when it was received by Omega Moving & Storage, Inc or 2) pay the full replacement value of the property. Carrier will not apply depreciation under this option. The charge of this option will be $15.00 per $1,000.00 of valuation..

The Cost Estimate that you receive from your mover MUST INCLUDE Full (Replacement) Value Protection for the articles that are included in your shipment. If you wish to waive the Full (Replacement) Level of protection you must complete the WAIVER of Full (Replacement) Value Protection shown below.
 

Full (Replacement) Value Protection is the most comprehensive plan available for protection of your goods. If any article is lost, destroyed or damaged while in your mover’s custody, your mover will, at its option, either 1) repair the article to the extent necessary to restore it to the same condition as when it was received by your mover, or pay you for the cost of such repairs; or 2) replace the article with an article of like kind and quality, or pay you for the cost of such a replacement. Under Full (Replacement) Value Protection, if you do not declare a higher replacement value on this form prior to the time of shipment, the value of your goods will be deemed to be equal to $6.00 multiplied by the weight (in pounds) of the shipment, subject to a minimum valuation for the shipment of $10,000. Under this option the cost of your move will be composed of a base rate plus an added cost reflecting the cost of providing this full value cargo liability protection for your shipment.

  • All items that will be protected ONLY in a NEW moving box, must be packed by an Omega Mover
  • An Omega Mover or your Moving Consultant may recommend items to be crated by wood for the proper protection while in transit; for example, high end TVs, all artwork, small motorized vehicles
  • Photos and Videos will be taken on the day of the move to provide confirmation of the condition of the items covered under the FVP plan
  • All items valued over $500 will itemized/inventoried separately from all other items included in policy, per advisement of shipper.  Additionally, the terms of Extraordinary (Unusual) Value must be accepted.

**Important Notice: Shipments valued over $100,000 will need an additional policy.

WAIVER of Full (Replacement) Value Protection. This lower level of protection is provided at no additional cost beyond the base rate; however it provides only minimal protection that is considerably less than the average value of household goods. Under this option, a claim for any article that may be lost, destroyed or damaged while in your mover’s custody will be settled based on the weight of the individual article multiplied by 60 cents. For example, the settlement for an audio component valued at $1000 that weighs 10 pounds would be $6.00 (10 pounds times 60¢)

Filing A Claim

Moving is inherently stressful. Fortunately, there are state guidelines and consumer protections put into place should there be any mistakes. Please note that if any claim is requested, the move must be complete and paid in full.

If any of your inventory is damaged or missing, you will have to file a claim to request repair of the item, or to be eligible for monetary reimbursement. Call the Omega immediately, and request a claim form or download the PDF from the website. Fill it out completely, sign it, and mail it to our office as soon as you have surveyed your entire inventory.

A claims adjuster will be assigned and he or she will review the claim and contact you via email or written correspondence. Please take note of the following details on the claims process:

  • All claims must be filed in writing, within nine months of the date of delivery to residence. If your goods were placed into storage-in-transit, and remain in storage in excess of 180 days, you have nine months from the date of conversion to permanent storage in which to file a claim.
  • Please do not discard or repair any items without prior authorization from this office, as we reserve the right to inspect all claimed items.
  • If you are claiming any damaged items which were packed, please indicate whether or not the carton was damaged. Please also save the packing material and carton for our inspection.
  • Please describe the nature and location of damages for each article claimed, and furnish repair estimates to support the claim.
  • Please provide the manufacturer’s name, the model, and the serial number of any appliances or electronics equipment claimed as missing or damaged.
  • Copies of original purchase receipts should be submitted with your claim for items requiring replacement.
  • Please make certain that all items you wish to claim are included on your claim form, and that the claim form does represent your complete and entire claim.
  • In order to process your claim, all moving balances needs to be paid in full and the claim forms to be returned to our office as soon you are 100% unpacked from your move.
  • Please note that you may only file ONE claim to our office within 9 months of your delivery but our office respectfully requests that we are notified to expect a claim within 90 days of your final delivery.
  • Please take your time to prepare all information that is necessary for your complete claim to be filed. In addition, we also request that ALL of the following materials be included with your complete claim forms.
  • Please complete forms in pen. Print all information clearly. Forms not legible will be returned to sender.
  • While reserving your right to file a claim, all forms but be submitted in a timely manner. Forms not submitted within 9 months of delivery will be returned to sender.
  • Please complete all blank lines. Forms not complete will be returned to sender.
  • Inventory Number must be identified for every item included in your claim.
  • Indicate color, type of material and type of lost or damaged item.
  • For mechanical items, you must provide the manufacturer's name, model number and serial number.
  • Indicate "CP" if items were packed by Omega and "PBO" if packed by yourself. Also notated if any external damage is visible to the article included in the claim.
  • Please review your terms regarding chips, dents, scratches, gouges and cracks , if selected Waiver of Full Protection.
  • Please do not fax photos. They can be mailed or emailed to [email protected].
  • Please make sure to unpack all of the household good items, once claim forms have been submitted to our office there will be no additions made to the claim forms.
  • Please be advised that your former Job# will now be your claim#.
